Important Note:  This pre-enrollment form merely begins the process by letting us know about you and your child. We’ll get back to you soon after your form is submitted. All admission and enrollment forms must be completed and the enrollment fee paid prior to your child’s first day of attendance. Based on availability and openings, our facility enrolls children ages six weeks to five years of age.
